You must complete this form (
please add your email
address to the top of the form) and then mail the form with a
check for the total amount due to GLRSA at PO Box 5811, Lafayette, IN
47903. We will collect the forms once a week from the post office and then
submit the orders to Blumling Design. Once the decals are ready for pick
up, you will receive an email from GLRSA letting you know your order is
ready and when/where you can pick up your order. Email us at
